Why Do I Need To Give You My Real Name And Address?

There are several good reasons for this:

I will send you special gifts (CDs for example) in the mail (free to you) once you become a student.
How can you and I have a personal teacher / student relationship if I don't even know who you are?
I will see your real name and address anyway when you make the first payment to begin our lessons together, so why hide your real name and address when filling out the online guitar lessons evaluation form?
